The DOL has posted PDF versions of the current Form 9089 Application for Permanent Labor Certification, the Form 9141 Application for Prevailing Wage Determination, and the form instructions for each on the DOL website. You can access them here:

Good practice is to print these and keep a copy handy in a binder on your desktop for quick reference when drafting the online versions, and check the website periodically for updates. The PDF versions function as relatively compact “big picture” roadmaps for the online forms (which are spread over many online pages) that can be studied whenever a question arises about where various pieces of information that are pertinent to a PERM application should go. (For example, where to enter travel language or degree equivalency language, or a beneficiary’s non-work related qualifications.) The current version of Form 9089 expires on 10/31/2025 while the current version of Form 9141 expires on 7/31/2026.
