Version 11.0, Build 135

The following resolved issue has been added to CaseMap version 11.0 (Build 135), released April 08, 2015:

 

hmtoggle_plus1Cannot create a new case from a template

CaseMap now prompts/allows you to select a custom template when using the New Case Wizard to create new cases.
